This form will ask you for:

Customer Info:

  • Name
  • Address to which the garment should be returned
  • Phone Number
  • Email 

Buttonhole Info:

  • Total number of Buttonholes
  • Sizes of Buttonholes (between 22mm and 30mm - size 32-48 buttons)
  • Locations for Each Size Buttonhole
  • Color of Buttonhole Thread:  Best Match or Contrasting Color?
